I am the photographer/videographer for all media sourced from Michael S. Schwartz, MD. He has released all media that I would like to contribute to Wikimedia Commons via the CC-BY-SA-3.0 license. If there's anything I am missing in regards to the proper release and licensing of my uploaded work, please advise and I will have that taken care of promptly. I've got tons of quality digital media from all sorts of medical procedures, and I'd like to have all the wrinkles ironed out before I start "mass-contributing" to the community (I'd prefer to not have to deal with legality issues for every single file I upload *grin*). Thanks for clearing this up. You won't need to cope with "legality issues" on each upload. One e-mail to OTRS from dr. Schwartz, stating that you are permitted to upload his work (your pictures) under CC-BY-SA-3.0 should suffice. The resulting OTRS ticket should then be mentioned on each of those uploads. That's all! Advance permission for images

We appreciate the the work you and Dr Schwartz are doing to provide high-quality freely-available images. And to answer your question, yes, it should indeed be possible to do this without having to release each image individually. The text on the website looks good, except that of course it's not really applicable to images that are not posted on that site, and will never be. What I suggest you do is to record a one-off permission with our OTRS system that can be used to validate all future uploads. The OTRS record will be given a unique tag number, and when uploading a new image all you will need to do is to include on the image page the template {{OTRS|number}} where "number" is replaced by your OTRS tag number.

Could you send an email, please, to permissions-commons@wikimedia.org, confirming that Michael S. Schwartz, MD, owner of the website www.drmichaelschwartz.com, is the copyright owner of photographs taken by you, and that until further notice he authorises you to upload to Wikimedia Commons surgical and other photographs relating to his practise under the license CC-BY-SA-3.0, including photographs that have not been posted on the website. So that we have evidence of identity, the email should be sent from an address within the domain drmichaelschwartz.com. If you could post a note here or on my talk page when that has been sent, I will look for it in the OTRS system and let you know the tag number.

Hi Laz. The email has arrived, but I'm afraid it has not come from the drmichaelschwartz.com domain, so we have no way of validating the identity of the sender. Would you mind sending another one, please, from that domain? To avoid doubt, I suggest that you should copy this exact wording:

Yea, I'll see what I can do. I know that there is no active email hosting at the moment ... But would it suffice if we update the domain "whois" information to include drmichaelschwartz@gmail.com under the registrant contact information?
