The licenses require that an attribution is shown alongside any copies, including modified copies, excepting only copies in Wikimedia and Wikipedia:

The attribution must contain a hyperlink from the copy back to the Wikimedia webpage containing the original photo or video file.

For print and video formats, and anywhere else that hyperlinks are not possible, the webpage addresses (URL's) for the original file and the license agreement must be included as text credits.
