English: About half of US adults have one or more chronic health conditions such as heart disease, cancer, diabetes, and arthritis. If you have a chronic condition, self-management education (SME) can help you develop strategies and confidence to manage your condition day to day. There are a variety of SME programs available.
This 3 minute video describes the benefits of SME for people with chronic conditions. For more information about self-management education, go to www.cdc.gov/learnmorefeelbetter
