Micrograph showing urothelial dysplasia. H&E stain/CK20 immunostain. This case has significant mitotic activity without appreciable inflammation, nuclear hyperchromasia, and minimal maturation to the surface. The small nuclear size is more in keeping with dysplasia. By definition, carcinoma in situ (CIS) has no maturation to the surface. Also, CIS typically has nuclear enlargement (nuclei 4-5x the size of a benign resting lymphocyte). The full thickness CK20 staining confirms dysplasia; however, it is not useful for differentiating urothelial dysplasia from CIS. Related images
