ตามระเบียบส่วนราชการในพระองค์ว่าด้วยการใช้ การเชิญ การแสดงธงพระอิสริยยศและการถวายความเคารพโดยใช้ธงชัยเฉลิมพล พ.ศ. ๒๕๖๒ ได้กำหนดแบบธงพระอิสริยยศของสมเด็จพระนางเจ้าสิริกิติ์ พระบรมราชินีนาถ พระบรมราชชนนีพันปีหลวง ไว้ดังนี้

ข้อ ๖ ธงเฉพาะพระองค์ สมเด็จพระนางเจ้าสิริกิติ์ พระบรมราชินีนาถ พระบรมราชชนนีพันปีหลวง ได้แก่

(๑) ธงสมเด็จพระบรมราชชนนีพันปีหลวงใหญ่ มีลักษณะเป็นรูปสี่เหลี่ยมผืนผ้า กว้าง ๒ ส่วน ยาว ๓ ส่วน พื้นธงสีเหลือง ตรงกลางตอนต้น ๒ ใน ๓ ส่วนของผืนธง เป็นรูปครุฑพ่าห์สีแดงอยู่ตรงกลาง ตอนปลายตัดเป็นแฉกรูปหางนกแซงแซว ลึก ๑ ใน ๓ ส่วนของความยาวของผืนธง และมีพระนามาภิไธย สก ประดิษฐานที่มุมบน
