In brief, please remove both the date of birth, Birth City and photo from all wikimedia/wikipedia pages, expeditiously. I have become the victim of Identity Theft and fraud. As a famous and very public person, my birth city, birth date has been used in this identity theft/fraud and is currently under investigation. Documents were manufactured/manipulated/doctored using my personal information, publicly displayed worldwide via your pages... As for the photo, it is publicly displayed, used without my consent and/or authorization and generally is not very flattering... Should you require official police records/data indicative of this identity theft, I can provide. In the meantime, please remove immediately. Should you have any questions/concerns, please don't hesitate to phone and/or email. Thank you in advance for your expedience and understanding in this matter. - Keep: Image was taken in a public place (concert), which does not require the consent of the subject. The image is used in w:Michael Henderson. I doubt the article will be deleted, since the subject is notable--he has been featured in Ebony and other publications, such as books about Miles Davis. Kept: This photo does not provide any information about the birth date etc. It has been taken in the public, apparently during a concert. Please contact the support team if there are issues with the article about you at en-wp. (Your photo is stored at Wikimedia Commons, that is a different project than the Wikipedia.)
